EDUCATION: Bachelor’s Degree required. Master’s Degree preferred. LICENSURE/CERTIFICATION: Valid PA Nursing license. Current BLS certification. Current certification in specialty area preferred or eligible to sit for certification. EXPERIENCE: Three years Medical/Surgical nursing experience required. Prior management experience required. At least two-five years experience case management experience preferred. SKILLS: Must be able to use informed judgment, individual competence and qualifications to prioritize patient needs, seek consultation, and to perform duties for diverse patient populations. Must have effective interpersonal communication skills and able to work as part of a team, in order to effectively collaborate with physicians and other healthcare team members to provide appropriate care and instruction to patients and family members. Must be able to use computer and software applications (i.e.: outlook, work, excel, power point). Must be able to speak in front of groups.

Job Description:

The position of Director of Case Management is responsible for the operational oversight of hospital-wide Case Management Program. Operational oversight includes but is not limited to the collaboration, research, development, implementation, and revision and reporting of the case management program outcomes to hospital committees. Work with all appropriate members of the health care team; ensure coordination of patient care through the development and use of standards of care and nursing practices. Ensure that specific outcomes are achieved and that variances from the established and accepted parameters are evaluated and addressed as necessary. Provide supervision and support to Case Management Team. Additional responsibilities include fiscal planning and regulatory compliance. Analyzes current systems and variance to identify opportunities for improvement and works to promote quality of care through collaboration with members of the interdisciplinary team.
