Director of Case Management

Graduate of a school of nursing. BSN preferred RN licensed by the WV State Board of Nursing, CCM Preferred, Healthcar provider BLS Three years related management experience preferred with experience in Utilization Review, Quality Improvement and Case Management preferred. Demonstrated ability to work and communicate effectively with hospital and medical staff. BLS Healthcare Provider Course.

Responsible for the coordination and daily operations of the case management process. Monitors care of patients within case groups to ensure high quality and cost effective care is provided. Works with other hospital personnel to assure appropriate and timely discharge planning. Works to continue to improve and change case management per the needs of the facility, patients and physicians.
